Job Description

HOPE Community Academy is an accredited, public co-educational charter school comprised of primary, intermediate, and middle school levels. The entire school community seeks to provide a safe and trusting learning environment where each student is valued and challenged to grow academically, socially, emotionally, and physically.

The mission of HOPE Community Academy is to educate all students to the highest levels of academic and social standards with a focus on Hmong language and culture. As a vital part of the total learning community, HOPE Community Academy is called to educate students in an atmosphere enriched by shared Hmong and American values . . . peace, justice, and respect for one another. The school recognizes that a child’s healthy development is dependent upon learning and living out those values.

We are seeking a dynamic and experienced Special Education Transitions Teacher to oversee and lead our special education programs. The Special Education Director will be responsible for developing, implementing, and monitoring special education services to ensure the academic and social success of students with diverse learning needs. The successful candidate will collaborate with educators, parents, and community stakeholders to create an inclusive and supportive learning environment.
Responsibilities:
1. **Transition Planning:** Developing and implementing transition plans for high school students with disabilities as they prepare to move from school to post-school life. This involves collaborating with students, families, school staff, and community agencies to set goals and create individualized transition plans.

2. **Life Skills Instruction:** Providing instruction and support in developing independent living skills, vocational skills, social skills, and other functional skills necessary for post-school success.

3. **Career Exploration:** Assisting students in exploring career options, identifying interests and strengths, and connecting students with vocational training programs, job shadowing opportunities, internships, and work experience placements.

4. **Community Resource Coordination:** Connecting students and families with community resources and services that support transition goals, such as vocational rehabilitation agencies, job coaches, mentorship programs, and adult service providers.

5. **Post-Secondary Education Planning:** Assisting students in researching, applying to, and transitioning to post-secondary education options such as college, technical schools, or adult education programs.

6. **Advocacy:** Serving as an advocate for students with disabilities in ensuring that their transition goals and needs are met within the school setting and in the broader community.

7. **IEP Development:** Collaborating with IEP teams to ensure that transition goals and services are incorporated into students' Individualized Education Plans (IEPs) and monitoring progress towards these goals.

8. **Data Collection and Progress Monitoring:** Collecting data on student outcomes, tracking progress towards transition goals, and adjusting interventions and services based on student needs and progress.

9. **Professional Development:** Engaging in ongoing professional development to stay current on best practices in transition services for students with disabilities and to enhance skills in supporting students in the transition process.

10. **Collaboration:** Working collaboratively with school staff, community partners, families, and students to ensure a coordinated and comprehensive approach to supporting students with disabilities in their transition to adult life.


These responsibilities are aimed at helping students with disabilities successfully transition from high school to post-secondary life, ensuring they have the skills and support they need to achieve their academic, career, and independent living goals.


Working Environment:
• Duties are normally performed in a school/classroom environment; school building is a multi-level building that has an elevator and is accessible
• Position requires extensive interaction with children of all ages
• Duties may occasionally be performed on field trips away from school
• Duties may occasionally be performed in outdoor environments, such as athletic fields or playgrounds
• Duties may occasionally be performed in varying weather conditions, such as hot or cold weather.
• Possible exposure to loud and noisy locations (e.g. loud classrooms or cafeterias; children speaking or laughing loudly).

Qualifications

Qualifications:
• Education: A bachelor's degree in special education, education administration, or a related field is often required.
• Experience: Previous experience working in special education settings, such as a special education teacher or related role, is usually required. E
• Knowledge of Special Education Laws and Regulations: A strong understanding of federal and state laws governing special education, such as the Individuals with Disabilities Education Act (IDEA).
