Job Description

Special Education Coordinator
This position will be for the 2024/2025 School Year

We are seeking a passionate and dedicated Special Education High School Transitions Teacher to join our team at HOPE Community Academy. This role is crucial in supporting high school students with disabilities as they prepare for life after graduation. The ideal candidate will have a strong background in special education, experience working with transition-aged students, and a commitment to fostering independence and self-advocacy skills
Responsibilities:
- Develop and implement individualized transition plans for each student with disabilities, focusing on post-secondary goals, vocational training, independent living skills, and community integration
- Collaborate with students, families, special education staff, and community partners to support the transition process and ensure continuity of services
- Provide direct instruction and support in areas such as career exploration, job readiness, social skills development, and self-determination
- Facilitate opportunities for real-world experiences, such as job shadowing, internships, volunteer work, and community outings
- Monitor student progress towards transition goals, collect data, and adjust interventions as needed to promote positive outcomes
- Prepare students for IEP meetings, post-secondary planning, and adult service agency referrals
- Serve as a resource and advocate for students with disabilities, promoting their rights and inclusion in the community
- Stay current on best practices in transition services and special education laws and regulations

Qualifications:
- Bachelor’s degree in Special Education or related field (Master’s degree preferred)
- Valid teaching license with special education endorsement
- Experience working with high school students with disabilities, particularly in transition planning
- Knowledge of transition assessment tools, post-school outcome data, and adult service agencies
- Strong communication and collaboration skills to work effectively with diverse stakeholders
- Ability to develop and implement innovative and age-appropriate instructional strategies
- Commitment to fostering a supportive and inclusive learning environment for all students
- Patience, empathy, and a genuine passion for helping students with disabilities achieve their full potential
